Applicants are to submit Appilcation Form A or B (Gansho), and also all of the required documents which are enclosed in the pamphlet of Admission Information Book (Boshu-yoko).
Please note that the downloaded Form A or B cannot be accepted.

Affix a 240-yen postal stamp to a self-addressed envelope (A4 size, Kakugata-nigou), write the name and address to which the envelope is to be sent, and then send the envelope to us.

We take care of both Graduate School of Engineering, and Advanced Integration Science.
Be sure to write the detailed information on the envelope as below.
